Building on his family legacy (his father pioneered phakic IOLs), Zaldivar's motivation stems from addressing longstanding challenges like inaccurate phakic lens sizing, suboptimal patient satisfaction, and limited access to real-world evidence in ophthalmology. He has emphasized democratizing advanced tools for global clinics, while expanding data-driven insights beyond refractive procedures to areas like retina, glaucoma, and dry eye. Zaldivar's work has had tangible impacts, including the widespread adoption of ICLGuru—an AI-powered tool for phakic IOL sizing using ultrasound biomicroscopy (UBM) data. By late 2025, ICLguru was adopted by over 450 clinics in 60+ countries, serving thousands of patients monthly with high vault prediction accuracy, reduced sizing errors and complications like excesive vaulting, and broader eligibility for extreme cases.

Through RevaiCare, an AI-powered digital health platform, Zaldivar has compiled extensive datasets of patient-reported outcome measures (PROMs), offering real-world insights into IOL performance and patient satisfaction. A 2025 study validated its AI for postoperative predictions, fostering multi-center collaborations. Now active in over 20 countries, the platform is expanding standardized care across subspecialties like retina, glaucoma, dry eye, cataract, and refractive surgery—aiming to transform decision-making in eye health.
