A new study suggests that emotional wellbeing may play a significant role in determining how independently older adults with cataracts manage their daily lives. Researchers in China have found that both positive and negative “affect” are associated with care dependence in elderly patients with senile cataract, with self-regulatory fatigue emerging as an important psychological mediator.
The cross-sectional study, published in Scientific Reports, examined 452 patients aged 60 years and older attending the Ophthalmology Outpatient Department of the Affiliated Hospital of Guizhou Medical University, China. Participants completed validated assessments measuring positive and negative affect, self-regulatory fatigue, and care dependence, allowing investigators to explore the relationships between emotional state, psychological resilience, and functional independence.
Care dependence is a growing concern among older adults with visual impairment. While cataracts are best known for reducing visual acuity, they can also affect an individual’s ability to perform everyday activities, increasing reliance on family members and caregivers. Previous studies have linked visual impairment with depression and anxiety, but the mechanisms underlying these associations have remained poorly understood.
The researchers found that positive affect – characterized by emotions such as enthusiasm, engagement, and optimism – was associated with greater independence. Patients reporting higher levels of positive affect had lower levels of self-regulatory fatigue and higher scores on the Care Dependency Scale, indicating reduced care dependence. Conversely, negative affect – characterized in the study by “distress-related emotions such as upset, nervousness, and fear” – was associated with increased self-regulatory fatigue and greater dependence on others.
Self-regulatory fatigue refers to the depletion of psychological resources required for self-control, emotional regulation, and goal-directed behavior. According to the study authors, chronic visual impairment may place additional demands on these resources by forcing patients to devote greater cognitive effort to everyday tasks. Over time, this can leave individuals less able to maintain self-care routines and independent functioning.
The findings add to a growing body of evidence highlighting the importance of psychological health in ophthalmic populations. While cataract surgery remains the primary intervention for restoring vision, the authors suggest that emotional wellbeing may represent an additional target for improving patient outcomes. Screening for emotional distress and self-regulatory fatigue in ophthalmology clinics could help identify individuals at greater risk of functional decline before surgery is undertaken.
The study's authors also point to potential interventions. Strategies designed to enhance positive affect – such as mindfulness programs, gratitude exercises, or peer-support groups – could help to preserve self-regulatory resources and promote independence. Likewise, addressing negative emotional states through psychological support could reduce the risk of increasing care dependence.
