Keratorefractive lenticule extraction (KLEX) has revolutionized vision correction by utilizing femtosecond lasers to create and remove a lenticule from the cornea without flaps. While standard KLEX typically uses optical zones of 6.0 to 7.0 mm, recent studies suggest that enlarging the lenticule size could enhance outcomes for younger patients with larger pupils struggling with night vision. A case series explored a new optical zone of 7.7 mm, yielding excellent visual results and contrast sensitivity, advancing the customization of refractive surgery.
